To Register a Team and Players
at the Beginning of the Season:
All registration materials must be submitted to Division Vice President.
- Team Affiliation/Registration Form
- please complete this form for submission to the MSA for your team.
This form is available from the Downloads page.
- A completed MSA Amateur Player Registration
Form for every player on your roster - very important that the forms are signed.
This form is available from the Downloads page. Complete all applicable sections of this form
except the League Number, and the Team Number. Remember to
fill out the individual section and the team section. The player must sign and the team manager
also must sign. The Player
Registration Number is pre-printed on the Player Pass.
- A "Clear and legible" photocopy
of the driver's license or other official state identification
for every player (new and returning) on your roster - this
is for MSA, not for the player passes.
- A completed Player Pass for every
player on your roster. If a player is participating in 2 divisions,
they will need 2 player passes. Please follow the Player Pass Instructions
(available from the Downloads
page) to complete your player passes.
- A registration fee of $20.00 per player for
the Michigan Soccer Association (MSA) & $5.00
per player for MMMSL. Please submit only one MSA registration
check per team. The check is payable to Michigan Soccer Association.
$5.00 Check is payable to MMMSL.
- If you have a player who is participating in multiple
divisions, they also must complete the MSA Multiple Registration
Form. This form is available from the Downloads page. There is no additional fee for a multi-team registration.
The player must sign and both
team managers must sign.
Situation #1 - a player
registering for 1 team would need to turn in the
following:
- Player Pass -
write the Reg. # from the back of the card on the
front of the card; fill out only the PRIMARY
REGISTRATION portion on the back of the pass card
- MSA Amateur Registration
Form, $20.00 MSA registration fee - the
Player Registration Number comes from the player pass and $5.00 MMMSL Fee.
- Copy of driver's license
or other ID
Situation #2 - a player
registering for 2 teams would need to turn in the
following:
- Assume the player picks Over-30 as his
Primary team and Over-40 as his Secondary team
- Turn in the following three items to
the Over-30 Team Manager
- Player Pass (blue Over-30) -
write the Registration number from the back of the card on the
front of the card; put the Over-30 information
into the PRIMARY REGISTRATION portion on the back
of the card and put the Over-40 information into
the SECONDARY REGISTRATION portion on the back of
the card
- MSA Amateur Registration form, $20.00 MSA
registration fee and $5.00 MMMSL Fee - the Player Registration
Number comes from the blue Over-30 player pass.
- Copy of driver's license or
other ID
- Turn in the following two items to
the Over-40 Team Manager
- Player Pass (yellow Over-40) -
write the Registration Number from the blue Over-30 player
pass on the front of the card; put the Over-30
information into the PRIMARY REGISTRATION portion
on the back of the card and put the Over-40
information into the SECONDARY REGISTRATION
portion on the back of the card.
- Multiple Registration Form - the
Player Registration Number comes from the blue Over-30
player pass; have the Primary Team (Over-30) Team
Manager sign the form, then take it to the Second
Team (Over-40) Team Manager.

To Register A
Player During The Season:
You can add players to your roster during the season.
A player must be completely registered 5 days before
they can participate in a match. To add a player to your
roster, you must submit the following materials to Division Vice President:
- If the player is not
already on a team in another division,
then you must submit a completed MSA
Amateur Player Registration Form - very important that the
form is signed. If
the player is already on a team in another
division, then you must submit a
completed MSA Multiple Registration Form.

- A photocopy of the
driver's license or other official state
identification for every player (new and
returning) on your roster - this is for MSA, not
for the player passes.
- A completed Player Pass.
If a player is participating in 2 divisions, they
will need 2 player passes. Please follow the Player

(available from the Downloads page) to complete your player passes.
- If the player is not
already on a team in another division,
then you must pay a registration fee of
$20.00 for the Michigan Soccer
Association (MSA) and $5.00 fee for MMMSL. The $20.00 check is payable to Michigan
Soccer Association and the $5.00 check is payable to Mid Michigan Mens Soccer League.

Miscellaneous:
The Open Division is open to all players with no age restrictions (Players
under the age 18 must submit an approved release form). The Over-30
division is open to all players who will be at least 30 years old by
December 31, 2009. The Over-40 division is open to all players who will
be at least 40 years old by December 31, 2009.
A player may participate in more than one division
within the MMMSL. See above for the registration
procedures.
A player may not participate on more than one team
simultaneously within a division. If a player wishes to
switch teams must be properly registered with the new team.

Fees:
Total Financial Outlay for each team that is due to MMMSL in 2009:
$20 per player (MSA Fee)
$5 per player (MMMSL fee)
$10.50 per game (GLASRA scheduler fee)
$24.00 per game (Field fee, EL Complex)
Total Financial Outlay for each team that is due to referees on game day:
$60.00 per game for Open and Over 30
$54.00 per game for Over 40 (8 v 8)
